Mission Statement
The mission of the SUNY Poly Wellness Center is to foster a confidential, compassionate, and inclusive environment that provides medical treatment and counseling services and educates and empowers students to pursue optimal personal health and well-being to better achieve academic and lifelong success.
Wildcat Wellness Student Portal
Please submit all required health documents to the student portal by August 1 (Fall semester) and January 1 (Spring semester).
- Log in using your SUNY Poly email and password
- Enter your birthdate (MM/DD/YYYY)
- Click the “Forms” tab and upload your required documents or download forms
You may also choose to fax them to the number listed above, mail them to Wellness Center, 100 Seymour Road Utica N.Y. 13502 or drop them off at Campus Center Suite 217.

No clinical or mental health services provided.
